Discharge

Central Children’s Home will ensure that termination of services, whether voluntary or involuntary, is an orderly process. If a client is disruptive and a threat to themselves, other clients, or staff, Central Children’s Home will take action to contact the referring agency or legal custodian to have the client removed immediately.

Discharge planning begins at intake with the development of the client’s service plan, which includes input from Central Children’s Home staff, parents and/or legal custodian, referring agency and therapist, if applicable.

Discharge planning begins at intake with the development of the client’s service plan, and sets out the goals they need to achieve to be discharged. The service plan includes input from Central Children’s Home staff, parents and/or legal custodians, and, if applicable, the referring agency or therapist.

Discharge Procedure:

  1. The Service Plan Advisory Committee convenes weekly to discuss the service plan goals and objectives of each client. The Committee includes the Executive Director (or their designee), Social Worker, Campus Life Director, Residential Supervisors, designated Residential Counselors from client’s cottage, and, if applicable, client therapists.
  2. If the decision is made that the client is eligible or needs to be discharged, the Staff Social Worker will contact the referring agency that day or by the end of the next business day to inform them of the Committee’s decision that the agency plans to discharge the client, thereby giving them the opportunity to discuss the decision.
  3. At that time, the Staff Social Worker will designate a time frame for the client to be discharged, which allows sufficient time for the client and agency to prepare for departure and for arrangements to be made for the client’s care.
  4. Clients will only be discharged to the referring agency or legal custodians.
  5. The Staff Social Worker will complete the Discharge Summary to include the following items (at a minimum) within 30 days of discharge:
    • Date, time, and circumstances of discharge
    • Name, address, and telephone number of the referring agency or legal custodian to whom the client shall be discharged
    • Services provided and client evaluation
    • Recommendations for needed services
    • Provision of or referral for after care services
  6. Upon the client’s departure, the Staff Social Worker shall provide a copy of the following items to the referring agency or legal custodian:
    • Medication and copy of client’s Medication Administration Record
    • Medical records
    • Dental records
    • Educational records
    • Clinical records (as available)
  7. Upon the client’s departure, Central Children’s Home shall send all personal clothing and belongings with the client.
